Reporting a Claim

How to report a claim:

If you become aware of an incident to which this policy may apply, or if a claim is made, or a suit is filed against you, you should give immediate written notice to the company or any of its authorized agents.

Include in your written notice the following information: 

• Date, Time and place of incident;

• What happened and what professional service you performed;

• Details of alleged wrong doing;

• Name, address and age of the injured party; and

• Names of witnesses, including other treating physicians

• Policy number
